Registration form

• All fields must be completed. A form with missing details will not be passed on for review by the committee.
• If the form is approved by the committee, we will let you know.
• Because of the large number of requests, we will update you when the review is complete. There is no need to call.

Please attach to the form:

• Copies of the ID cards + ID appendix (sefach) of both spouses
• Pay slips
• Bank account statements for the last 5 months
• A letter of recommendation is advisable
